The Health Information Exchange (HIE) Bi-Directional Exchange measure is an alternative to attesting to the Support Electronic Referral Loops by Sending Health Information and the Support Electronic Referral Loops by Receiving and Reconciling Health Information measures. Attestation to this measure is optional.
ECs interested in participating in an HIE should contact Sevocity Support to begin the process of a new interface setup. Interface setup requirements and fees vary per request.
CMS defines a transition of care as the movement of a patient from one setting of care (hospital, ambulatory primary care practice, ambulatory, specialty care practice, long-term care, home health, rehabilitation facility) to another. At a minimum, this includes all transitions of care and referrals that are ordered by the MIPS eligible clinician.
In order to receive credit for this measure, the EC must be connected to an HIE and exchanging data as specified by the attestation statements for the performance period reported. Credit will not be given to ECs that are in the process of establishing these connections.
