About Patient Reminders
The Patient Reminders tool is used to create and send communications to patients based on Health Maintenance Disease Management Reminder List Report criteria. This feature is designed to improve patient health outcomes and increase patient engagement by creating a streamlined workflow to manage population health and document communication with patients.

Users with an Access Level of Restricted Chart Entry or Full Chart can access Patient Reminders.

Clinic Administrator privileges are required to create or edit Health Guidelines.
- Items available in the Health Guideline Reminder list are enabled by the Clinic Administrator in the Health Guidelines/Disease Management tool by selecting the Include in Reminders Tab checkbox for the guideline
- Letter Templates are customizable at the user or CLINIC user level. Go to Tools >Preferences > CLINIC or [user] > Letter Templates > Begin Edit
- Patient portal message favorites for Patient Reminders can be created by Clinic Administrators by going to Tools > Preferences > CLINIC > Clinic Wide Form Data > Begin Edit and selecting Messaging from the Tab list
From the Patient Reminders tab, users have the ability to:
- Create patient reminders in the form of a letter
- Document patient reminders conducted over the phone
- Create and send patient reminders through the patient portal Secure, online tool that enables patients to access their health information and communicate securely with the clinic to an individual patient or multiple patients simultaneously
Patient Reminders can also be used as an administrative management tool by customizing Health Guidelines criteria to create a more personalized clinic experience for patients. Examples include:
- Patient birthday acknowledgments
- Back-to-school screening invitations
- “All Patient” contact list to notify of clinic closures or change in business hours
From the Sevocity desktop, click the Patient Reminders tab. Creating a patient reminder list requires four steps:
- Select a Health Guideline Reminder
- Select Patients to Include:
- All patients
- Patients seen in the past 2 years
- Patients seen in the past year
- Select a Reminder Method:
- Letter
- Phone
- Portal
- Click Run
All eligible patients will display in the list along with a Total Patients count. The list will display patient portal enrollment indicator (), patient Last Name, First Name, DOB, Account #, Preference for reminders, and date of Last Sent reminder.
The steps to create a patient reminder will vary depending on the reminder method selected.
Select a patient and click Open Chart to view the patient's chart prior to creating a reminder.
