Export CCDs

CCDs contain the following information per patient file:

• Clinic Demographics: Clinic name, address, and phone
• Provider Name and NPI
• Patient Demographics: name (first, middle, last, suffix), birth name (first, middle, last, suffix), birth sex, date of birth, time of birth, marital status, race(s), detailed race(s), ethnicity, primary language, account number, SSN, address (street, city, state, zip code), home phone, home phone extension, work phone, work phone extension, and cell phone
• Patient Clinical Information: allergies, medications, problems, procedures, e-lab results, encounter diagnoses, functional status, immunizations, plan notes, reasons for referral, smoking status, vital signs, mental status, assessment notes, goals, and health concerns

Go to Tools > Administration > Export CCD(s) to open the CCD Export Schedule Manager. The Schedule Manager displays scheduled exports and completed export jobs that are ready to download.

1. Go to Tools > Administration > Export CCD(s)
2. Click Create New Schedule

From the Create New CCD Export(s) window:

3. Select a Provider
   1. The provider selected is assigned as the author of the CCD
4. Select the Patient(s) to Include:
   1. Individual: will create a CCD for a single patient
      1. Click Select Patient to search for and select the patient
   2. All Patients: will create CCDs for all active patients
   3. Patients Seen: will create CCDs for patients seen in the specified time frame
      1. Click From and populate the From and To date fields or click In and select a time frame from the list
5. Select the Export Frequency
   1. Now: will queue the export to run when the export request is created
   2. Scheduled: will queue the export to run a single time at a specified date and time or on a recurring basis
      1. Click One Time and populate the date and time fields or click Recurring and select a month interval and day and populate the time field
      2. Export requests can only be scheduled to run between 6:00p and 4:00a Central Standard Time
6. Type an Export Name
7. Optional: click to select a Download Directory, a location on a local computer or directory where the export file will be saved upon download
   1. The download directory location can also be specified when the file is downloaded
8. Optional: click Add in the User(s) to notify when export completes section to select other Clinic Administrators within the clinic to notify upon the completion of the export job
   1. Notifications are received in the Messages inbox
9. Click Next
10. Review the export request details and click Schedule

The scheduled export will display in the CCD Export Schedule Manager.

When a scheduled export request is completed, the file will become available for download in the CCD Export Schedule Manager.

• CCD export files will be available for download up to 30 days after completion
• Each CCD export file is available for download only once

Download a CCD Export File

1. Go to Tools > Administration > Export CCD(s)
2. Click in the Download column for the export file to be downloaded
3. Optional: click to select a Download Directory
   1. If the download directory location was specified when scheduling the export request, the location will be pre-populated
4. Create and verify a password by populating the Create password and Confirm password fields

   1. The password is used to encrypt the export and will be required to open the file

5. Click Start Download

   

6. Click Yes to confirm that downloading the export file will make the file unavailable for future downloads

The CCD export will be saved as a compressed (zip) file in the specified download directory.
