Patient List Report

1. From the Reporting Tool, go to Reports > Patient List
2. Select a Birth Sex or leave defaulted to All birth sexes
3. Populate Age From and Age To fields to specify the age range for the patients
   1. Age can specified in Years or Months (up to 24)
4. Select one or more diagnosis codes from the following options:
   1. Select a code from the Favorite Assessment (ICD‑9) list or click Search Master List to select from all ICD‑9 codes
   2. Select a code from the Favorite Assessment (ICD‑10) list or click Search Master List to select from all ICD‑10 codes
5. Select a Medication Name or a Medication Class:
   1. To add a Medication, click Retrieve to produce a selection list of all medications currently active for the clinic's patient population and select a medication from the list
   2. To add a Medication Class, click Retrieve to produce a selection list of all medication classes currently active for the clinic's patient population and select one or more medication classes from the list
6. Optional: select Lab Result criteria
   1. To add a Lab Result, click Retrieve to produce a selection list of all lab result fields currently active for the clinic's patient population and select a result field from the list
   2. A Low or High result range can be specified for each lab
7. Select a Sort By value for the report output
8. Select Patients to include
9. Click Create Report

Note

	Reports with an extensive amount of data are unable to be generated as a PDF and will require the user to export the report to view the data.

The Patient List report will display the Patient Name and patient demographic data, diagnosis code Description detail, and medication Strength/Form.

Generated report data can be exported and saved outside of Sevocity to a destination selected by the user, such as a network drive or local machine.

1. From the report, click Export
2. Select a location to save the file and type a File Name
3. Click Save

   1. The data will be saved as a comma delimited file (.csv)

      

4. Click OK to acknowledge the export is complete

   

Why is the Export Button Disabled?

The Export button becomes enabled only if there is data in the generated report.
