Demographics Report

1. From the Reporting Tool, go to Reports > Demographics
2. Select a Clinic Provider or leave defaulted to All clinic providers
   1. Selecting a Clinic Provider filters the report to display patients assigned to the selected provider
3. Select a Birth Sex or leave defaulted to All birth sexes
4. Populate Age From and Age To fields to specify the age range for the patients
   1. Age can specified in Years or Months (up to 24)
5. Optional: select one or more items from the Exclude criteria to remove this information from the report output
6. Select Patients to include
7. Click Create Report

Note

	Reports with an extensive amount of data are unable to be generated as a PDF and will require the user to export the report to view the data.

The Demographics report will display Patient Name, DOB, Birth Sex, and Clinic Account #, Address, Email , and Cell Phone unless excluded in the report criteria.

Generated report data can be exported and saved outside of Sevocity to a destination selected by the user, such as a network drive or local machine.

1. From the report, click Export
2. Select a location to save the file and type a File Name
3. Click Save

   1. The data will be saved as a comma delimited file (.csv)

      

4. Click OK to acknowledge the export is complete

   

Why is the Export Button Disabled?

The Export button becomes enabled only if there is data in the generated report.
