Patient Portal Report

1. From the Reporting Tool, go to Reports > Patient Portal Report
2. Select one of the following report options:
   1. All Patients: displays a list of all patients enrolled in the portal
   2. Patients added to portal from: displays a list of patients added to the portal within the date range specified
   3. Patients last login date from: displays a list of patients whose last log in for the portal was within the date range specified
   4. Patients with QR code generated from: displays a list of patients for whom a QR code was printed or sent to the portal within the date range specified
   5. Patients never logged in: displays a list of patients with portal accounts who have never logged in to the portal
3. Click Create Report

Note

	Reports with an extensive amount of data are unable to be generated as a PDF and will require the user to export the report to view the data.

The Patient Portal report will display the Date Enrolled, Last Logged In date, if the patient has opted in to receive notifications (Opt In Email), and if a portal QR code has been generated for the patient.

Generated report data can be exported and saved outside of Sevocity to a destination selected by the user, such as a network drive or local machine.

1. From the report, click Export
2. Select a location to save the file and type a File Name
3. Click Save

   1. The data will be saved as a comma delimited file (.csv)

      

4. Click OK to acknowledge the export is complete

   

Why is the Export Button Disabled?

The Export button becomes enabled only if there is data in the generated report.
