Release Notes December 31, 2025

The following changes have been made to the Coding tab to align with annual CPT coding updates:

Remote Physiologic Monitoring – Digitally Stored Data Services

• Updated the description for 99453 to clarify initial setup and patient education.

• Added a new code, 99445, for device supply and data transmission when services occur 2–15 days in a 30-day period.

• Updated the description for 99454 to reflect device supply and data transmission for 16–30 days in a 30-day period.

Remote Physiologic Monitoring – Treatment Management Services

• Added 99470 to the top of the Evaluation & Management code list for first 10 minutes of monthly treatment management time.

• Updated the description for 99457 to clarify first 20 minutes of monthly treatment management time requiring real-time patient or caregiver communication.

• 99458 functionality remains unchanged:

  • Available only when 99457 is selected

  • Disabled when 99470 is selected

Remote Therapeutic Monitoring (RTM)

• Added new device supply codes to support 2–15 days of monitoring:

  • 98984 – Respiratory system

  • 98985 – Musculoskeletal system

  • 98986 – Cognitive behavioral therapy

• Updated descriptions for existing codes to reflect 16–30 days of monitoring:

  • 98976 – Respiratory system

  • 98977 – Musculoskeletal system

  • 98978 – Cognitive behavioral therapy

• Removed 98980 from this category (now available under RTM Treatment Management Services).

• Added a new category specifically for RTM treatment management services.

• Added new Evaluation & Management codes:

  • 98979 – First 10 minutes of monthly treatment management time

  • 98980 – First 20 minutes of monthly treatment management time

• Added 98981 as an optional checkbox for each additional 20 minutes:

  • Disabled by default

  • Enabled only when 98980 is selected

  • Units are pre-populated as X 1 and can be adjusted by the user

  • Follows the same logic used for 99458 in RPM Treatment Management Services

Expanded Use of G2211

The G2211 add-on code for visit complexity can now be used in more care settings. In addition to office and outpatient visits, G2211 is now allowed for evaluation and management (E/M) services provided in a patient’s home or temporary private residence.

• Added the following codes to the top of the Evaluation & Management list:

  • 92507 – Individual speech and language therapy

  • 92508 – Group speech therapy
